Monogram Health, is a value-based provider of in-home, evidence-based care and benefit management services for patients living with polychronic conditions, including chronic kidney and end-stage renal disease. Our innovative model delivers specialty care for patients where it previously did not exist. Monogram is dedicated to improving access to health care for those who have been historically underserved. Our high-touch, in-home model is driven by physicians, nurse practitioners, registered nurses, social workers, pharmacists, and care management teams. Our providers and clinical experts are further bolstered by utilization management. Monogram currently serves patients in 4,000 US cities across 34 states.
