PeaceHealth is seeking a Nurse Practitioner - Geriatric: Home visit based in the Eugene community.

The PeaceHealth Gerontology Institute is seeking a Full Time Adult Gerontology Nurse Practitioner (AGNP), Adult Nurse Practitioner (ANP), Gerontological Nurse Practitioner (GNP) or Family Nurse Practitioner (FNP) with an interest in geriatric medicine to work as a team member.

Nurse practitioners, working with full practice authority, have the option of practicing within a home visit environment, managing a geriatric patient population.
Nurse practitioners with a passion for improving and/or maintaining the quality of life for the older adult are welcome to join our skilled, collaborative, and compassionate team of geriatric specialists.

For the past decade, PeaceHealth West Oregon Network has been developing innovative and comprehensive senior health programs to meet the challenge of a growing population of older adults in the community, especially the frail and vulnerable. We are dedicated to exceptional medicine and compassionate care. The Gerontology Institute is committed to expanding geriatric knowledge, applying proven geriatric best practices, spreading geriatric expertise throughout the community, and improving access to care.

Within the Senior Health & Wellness Center (SHWC), an outpatient geriatrics clinic which provides comprehensive primary and consultative care for adults age 65 and older, nurse practitioners serve as primary care providers and consultants or home visit providers to meet the needs of an expanding older adult population. In addition to comprehensive primary care, a multidisciplinary team at SHWC, operating collaboratively, also offers care management, counseling, diabetes education, foot care, geriatric specialty consultations, memory and cognitive evaluations, and medication education/management for chronic diseases.
