Case Manager

Lighthouse Behavioral Wellness Centers, a Community Mental Health Center in Southern Oklahoma, is searching for a Case Manager who embodies the agency's core values of Hope, Dedication, and Community.

Requirements:

Bachelor's degree in Psychology, Social Work, Counseling or similar mental health related field.
Must successfully complete (and maintain) case manager certification within 3 months of employment.
Examples of work performed:

Provide case management services to promote recovery principles and supportive services to individuals with serious mental illness
Must be able to function as part of an integrated treatment team
Submit required billing, reports and forms;
Manages assigned caseload within a team approach to care;
Completes intakes for new consumers accessing services.
May provide wellness services to include assisting consumers in meeting nutritional and physical activity goals and tobacco cessation.

Assistant Program Manager/Care Coordinator

Lighthouse Behavioral Wellness Centers, a community mental health center in Southern Oklahoma, is looking for a dynamic leader in our Children's behavioral health program in Pauls Valley, Oklahoma. The Assistant Program Manager supervises clinical staff in multiple counties and provides wraparound services to children and families within our Systems of Care/Health Home program.

RESPONSIBILITIES:

A. Provide daily supervision/support to clinical staff to ensure that quality of services and clinical documentation are completed accurately and efficiently.
B. Participates in the planning and delivery of wraparound services to consumers.
C. Provide information and on site coaching to direct care staff in the Systems of Care program to improve services for families in the wraparound model.
D. Represents the agency and assists Project Director with Community Meetings and Committees/Teams when needed.
E. Functions as an agency liaison to other agencies such as Child Welfare, DHS, OJA, ODMHSAS, and OHCA.

QUALIFICATIONS:

A. Completion of the curriculum requirements of a Master's or Bachelors degree in a mental health field from an accredited university and three (3) years experience in a treatment setting. Must have experience with children related to mental health treatment. One (1) year of supervisory experience in mental health services preferred.
B. Case Management certification and Care Coordinator Wraparound training shall be completed within three (3) months of date of employment and maintain same during period of employment.
C. Knowledge of current social and economic problems, principles of family economics and budgeting, human relations, community, state and national health and welfare resources, social assistance requirements, and psychological terminology.
D. Ability to work effectively and courteously with people, and to exercise good judgment.
E. Must possess valid drivers license, reliable transportation, and be eligible for agency insurance.
F. Must have excellent written and communication skills.

Must maintain a valid drivers license and personal automobile insurance.

What is Lighthouse Behavioral Wellness?

Lighthouse Behavioral Wellness Centers is a Community Mental Health Center located in South Central Oklahoma. We cover nine counties: Carter, Love, Garvin, Murray, Pontotoc, Seminole, Bryan, Johnston, and Marshall. Lighthouse was recently awarded a SAMHSA expansion grant to become a Certified Community Behavioral Health Clinic (CCBHC). Our main location and administrative offices are in Ardmore, Oklahoma with outpatient clinics in Ardmore, Ada, Durant, Pauls Valley, Seminole, and Tishomingo. Lighthouse provides outpatient treatment to individuals with behavioral health and substance use issues. Lighthouse is a trauma-informed agency and focuses treatment around an individual’s needs and preferences. We provide integrated treatment and addresses both behavioral health and physical health issues. We employ psychiatrists, nurse practitioners, registered nurses, counselors, case managers, and peer support staff. We provide services to adults, adolescents, and children. Our children's program focuses on wraparound services within a Systems of Care model.

Lighthouse also operates a Community Based Structure Crisis Center in Ardmore for adults experiencing a mental health crisis. We also operate 2 Urgent Recovery Centers (Ardmore and Durant) and will soon be expanding to a third location (Ada). We provide 24 hour crisis response to our nine county area.
