Licensed Practical Nurse

EVERY OTHER WEEKEND REQUIRED

Summary: Under the supervision of a Registered Nurse, provides resident care using the nursing processes of assessment, planning, implementing, and evaluation.

Responsibilities:
-Effectively manages the care of assigned residents using knowledge, clinical skills, judgment and decision making skills for the specific area, under the direction of a Registered Nurse.
-Delivers resident care utilizing guidelines of care and appropriate resources, including personnel, services, supplies, and equipment.
-Reviews, evaluates and takes appropriate action regarding results of diagnostic tests.
-Administers medications according to regulatory standards for long term care.
-Assists the RN in performing the nursing assessment to identify and prioritize physical psychosocial and cultural needs in order to develop a plan of care. -Developing care plans for resident admissions as assigned.
-Evaluates and reassesses nursing interventions based on interdisciplinary plans of care and makes revisions as appropriate.
-Assists with the development and implementation of a patient/family education plan. Documentation to meet compliance with regulatory standards.
-Utilizes resident care supplies and equipment in a fiscally responsible manner.

Minimum Qualifications:
-Graduate of an LPN program.
-Previous experience not required, but preferred.
-Current licensure to practice as a LPN in the state of Georgia.
-Nursing skills as defined in the law governing the practice of nursing in Georgia.
-Current American Heart Association BLS Certification is required.
-Exhibits positive interpersonal skills. Displays good judgment and critical thinking skills.

Registered Nurse/Infection Control

Southeast Georgia has an opportunity for a full time RN/Infection Control to join our Senior Care Center - Brunswick team!

**SIGN ON BONUS AND RELOCATION ALLOWANCE AVAILABLE TO QUALIFIED CANDIDATES**

Summary: Monitors compliance related to practices associated with the prevention and control of healthcare associated infections. Abstracts clinical data on a retrospective basis as well as utilizing current data related to Infection Control issues. Maintains CDC compliance for facilities that have more than 100 residents should have a full-time team member in this role.

Responsibilities:
-Coordinates Quality Assurance and Infection Control activities for the long term care facility. Abstracts or obtains abstracted data-whether automated or manually, ensures data entry is completed timely, exports data to MyInnerview and/or GMCF, prints reports, researches edits and re-exports data. Develops, implements, and monitors surveillance of nosocomial infections and other adverse events to implement quality improvement activities designed to improve patient outcomes
-Provides comparative data for Disease Management efforts to identify best practices. Prepares reports of quality indicators & submits to appropriate committee. Keeps staff and committees aware of publicly reported data reports.
-Analyzes data and utilizes performance improvement tools and techniques, such as graphs, and charts for quality and performance improvement reports to -Administration and the Quality Assurance Committee. Utilizes graphics when appropriate in presentations. Utilizes accurate methodology in the selection, risk adjustment and analysis of indicators
-Manages data for required compliance with regulatory agencies, e.g. restraint data, falls, and pressure ulcers. Ensures all components are tracked and trended. Provides reports to appropriate committees and outside agencies as required at assigned frequencies. Summarizes findings, identifies trends and makes recommendations in defining areas for opportunity for improved efficiency and quality of care.
-Assists in the development, review, and revision of policies and procedures based on scientifically valid infection control prevention and control measures that protect the health and safety of patients and caregivers.
-Assists in the implementation and integration of the organizations Quality -Improvement and Patient Safety Plan.
-Intervenes directly to prevent transmission of infection by the identification and implementation of control measures during outbreaks or clusters of infectious diseases.
-Communicates Infection Control information and interventions to the Health System Infection Control Coordinator.

The ideal RN/Infection Control candidate should posses the following minimal qualifications:
-Graduate of a Diploma, Associates Degree or Bachelors Degree Nursing Program
-Current RN license to practice in the State of Georgia
-Nursing skills as defined in the law governing the practice of nursing in Georgia.
-Current American Heart Association BLS Certification is required. Uses good judgment. Exhibits positive interpersonal skills.
-Excellent computer skills a necessity MS Office, data entry, graphic development and presentations. Requires the ability to manage programs and projects. Knowledge of statistical analysis & display, data management, basic coding. Demonstrated ability of excellent interpersonal, written and communication skills.

Certified Nursing Assistants (Full Time)

Southeast Georgia Health System is seeking full time Certified Nursing Assistants to join our Senior Care Center team at our Brunswick campus.

Responsibilities:
Assists patients/residents with activities of daily living, provides basic care, and assists in the maintenance of a safe and clean environment under the supervision of licensed nursing staff. Other duties as assigned.

Shift: 7am-7pm or 7pm-7am

Qualifications:
High School Diploma or equivalent preferred, but not required.
Current Georgia Certified Nursing Assistant OR temporary Certified Nursing Assistant certificate required.
Current American Heart Association BLS Certification is required.
Positive interpersonal communication skills.
Demonstrates a cooperative attitude by being tactful and sensitive while using sound judgment when relating to others.
Must demonstrate proficiency in taking and recording vital signs and obtaining blood glucose values.
