About the Organization
AxessPointe is a group of federally qualified health centers (FQHCs) located in Northeast Ohio. Our mission is to provide a quality, affordable, and compassionate health home for every patient, every time, in all communities that we serve.
Our three health centers provide medical, dental, and behavioral health services to our patients. We also have on-site 340B pharmacies, food pantries, and personal hygiene closets. Our practice model includes additional support services for our patients through community outreach events and our community healthcare workers.
