About the Organization
Founded in 1994, Element Care PACE was established as a joint venture of the Lynn Community Health Center and Greater Lynn Senior Services to bring the Program for All Inclusive Care for the Elderly to seniors in our community. Today, Element Care PACE has 8 PACE Adult Day Health Centers and two alternate care settings covering the North Shore, Merrimack Valley and the Northeast region of Middlesex County in Massachusetts.
About the Community
Participants are 55 and older. We have centers on the Northshore and Merrimack Valley of Massachusetts and will be opening a new center in Brighton, MA soon. Managing multiple chronic conditions is no easy task in todays fragmented health system. All too often different specialists who are unaware of each others treatments may prescribe unnecessary, duplicative and potentially harmful tests, medications and x-rays or other imaging scans. At Element Care, our team coordinates all of participants care including appointment setting, transportation and follow up tasks, so that the patient can focus on living their life.
