Our goal is to transform how health care is delivered to the most medically vulnerable. We are a risk-based provider group that partners with health plans to provide high-touch, longitudinal care for chronic, complex patients.

Our doctors, nurse practitioners and physician assistants deliver care to eligible plan members exclusively through house calls, with visits tailored to wellness and disease management, post-acute management and urgent care.

Landmarks Complexivist care model means our providers are supported by fully employed multidisciplinary clinicians, including behavioral health specialists, palliative care practitioners, pharmacists, nurse care managers, dietitians, social workers and ambassadors.

We started seeing patients in upstate New York in 2014 and have quickly grown to be one of the nations leading risk-bearing medical groups focused exclusively on caring for the most complex and chronically ill populations.
