About the Organization
Northwest Human Services meets the needs of community members, regardless of social or economic status, by providing comprehensive medical, dental, mental health, and social services with respect, compassion, and acceptance of cultural and linguistic diversity.
As long as there is need in our community, we will continue to be a safety net providing health care and social services.
Northwest Human Services is a fiscally responsible nonprofit 501(c)(3) organization committed to serving the needs of disenfranchised citizens in the Mid-Willamette Valley. Our comprehensive social services programs and medical services provide assistance to disadvantaged individuals and families who would not otherwise have access to essential care.
Northwest Human Services West Salem Clinic and Total Health Community Clinic are full spectrum family practice sites offering services ranging from pediatrics to geriatrics. Services involve management of acute illnesses to care for the chronically ill. There is limited hospital call or admissions and involves oversight of pediatrics inpatient only. Call is limited to Hospital only and Mid-Level providers manage outpatient call with Physician back-up.
We offer pre-natal care to patients up to 28 weeks and limited procedure expectations. Our sites offer fully integrated Medical, Dental and Mental Health services.
Our sites are PCPCH Certified (Patient Centered Primary Care Home Program), Joint Commission Accredited; approved NHSC (National Health Service Corps) approved sites with a HPSA Score of 16 and an FQHC deemed facility.
As an FQHC, we serve the underserved population, offer a sliding fee scale, provide comprehensive services, have an ongoing quality assurance program, and have a governing board of directors.
