Responsibilities

Provide direct medical care within the outpatient clinic
Works collaboratively with patients to meet improve their health and well-being
Delivers evidenced based care that supports the Triple Aim of improved health outcomes
Exceptional patient experience and efficient use of resources is required
This position will practice in outpatient with call rotation for phone triage
100% outpatient care
Call rotates with other IM physicians
Functions appropriately as part of care delivery team through efficient use of resources and skills, both their own and those of other provider and staff
Chart in electronic format in accordance with licensure and scope
Actively participates in multidisciplinary team approach to case management
Quality improvement utilizes quality improvement tools to develop processes that promote continuous improvement in delivery of patient care, patient outcomes or workflows

Education

This position requires a Medical Doctorate degree (M.D.), Doctor of Osteopathy (D.O.) or foreign equivalent from an approved school of medicine
Board Eligible/Board Certified (BE/BC) Physician in Family Medicine or Internal Medicine
Board certification in Internal Medicine preferred within two years of employment

Qualification/Skills

Experience
Minimum two years experience preferred practicing medicine in a clinical setting, but will consider candidates directly out of residency.

Skills
Excellent communication skills verbal and written. Highly organized, self-directed and motivated. Creative and innovative problem solver

Licensure/Certifications
Board Eligible/Board Certified (BE/BC) Physician in Family Medicine or Internal Medicine
Board certification in Internal Medicine preferred within two years of employment
Must have current, unrestricted Minnesota State Medical Licensure

The city of Coon Rapids is a fast growing community on the Mississippi River just 20 minutes north of Minneapolis-St. Paul, and the metropolitan area gateway to the northern part of the state.
