UnityPoint Health Meriter is actively recruiting for a Family Medicine Nurse Practitioner to join our West Washington Clinic team! .

This position will:

Provide ambulatory services for patients which include but are not limited to consults, urgent appointments, evaluation and follow up visits, transitional care of patients being discharged from hospital, medical wellness visits.
Order and interpret appropriate diagnostic and laboratory testing.
Educate clinic patients and/or families about preventative care, medical issues, and use of prescribed medical treatments and/or medications.
Consult comprehensive or episodic health history and physician assessment of patients with acute, chronic, and/or potential health problems.
Establish medical diagnoses, and treatment plan based on history and physical assessment.
Work collaboratively with physicians, care coordinators, nurses, social work, pharmacists, and behavioral health specialists.

Work Schedule: Varies (Monday-Friday) FTE: 0.5

Qualifications Required:

Graduation from an accredited Nurse Practitioner (APNP) program
Current and valid State of Wisconsin APNP license
Valid and established DEA license number
Years of experience: 3+ years preferred, but not required.
