Overview of Opportunity:

Responsible for overseeing the Institutional Special Needs Plan (I-SNP) model of care in a designated CareMore region. Primary duties may include, but are not limited to: Mentors, teaches, and supervises CareMore Touch Nurse Practitioners. Provides oversight for hospitalized and skilled nursing facility patients. Manages end of life care for CareMore Touch members. Covers call as needed. Provides primary care for a limited number of members. Promotes and grows I-SNP product, CareMore Touch, designed for Medicare beneficiaries living in Nursing Homes, Assisted Living and Board and Care Settings. Hires, trains, coaches, counsels, and evaluates performance of direct reports.
