About the Organization

Bluestone Physician Services was founded in 2006 to provide on-site primary care to chronic-care patients within a variety of facilities, most often Assisted Living, Memory Care, and Group Homes. Today we serve thousands of Geriatric and Integrated Care patients across multiple states. Bluestone is one of the largest providers of residential primary care in the nation. We expanded to Milwaukee and Madison, Wisconsin in 2014 and Tampa and Orlando, Florida in 2015.

For our health plan customers, we also provide Coordinated Care through community-based teams of RNs and social workers with medical provider support and consultation.
