At GraceMed, we believe our communitys potential is immeasurably expanded by improving the health of our most medically underserved residents. Thats why were dedicated to delivering nothing less than our very best care to thousands whose access to healthcare is limited either by their economic resources or the neighborhood where they live.
The quality of your care is of the utmost importance to all our providers at GraceMed. As a Federally Qualified Health Center (FQHC), our care must meet or exceed measurable standards of care established and monitored by HRSA.
Treating your whole health is important to us, too. We are certified under the standards of the National Committee for Quality Assurance (NCQA) as a Patient-Centered Medical Home. This recognition is held by those practices which meet the NCQA criteria for providing continuous, whole person care in an integrated, team-based system.
As we serve more of the underserved, families are strengthened. Hospital emergency rooms spend less time and resources providing non-emergency care. And area employers benefit from a healthier workforce.
