Fairview Health Services is seeking 2 (.6 FTE) Nurse Practitioners (NPs)to join a primary care practice at Fairview Clinics -Rogers to provide same day care and assist with growth of thepractice The NP's will collaborate closely with the primary providers on site to offer patients a seamless transition from same day appointments to establishing care and will assist with management of chronic health and preventative medicine.

We have a fully integrated Electronic Medical Record (EPIC)
Hours of the position are 10:00am-7:00pm 3 days per week.

As part of our participation in the Pioneer Accountable Care Organization (ACO), the physicians use a team approach in managing clinical care. The site is using a model of care, Care Model Innovation, for their patients which actively looks at and changes clinical processes to maximize efficiency, resources, and clinical care.

Position Qualifications:

A qualified candidate will be a graduate of an accredited school of nursing and formal Family Nurse Practitioner training program. Have a current Minnesota APRN license and be approved or eligible by the Minnesota Board of Nursing for prescription writing privileges. Be eligible or certified throughANCC or AANP and meet threshold criteria/qualifications for Credentialing and Privileges. Ability to obtain and/or maintain DEA certification for the State of Minnesota.

Preferred Qualifications:

Previous primary care experience is preferred but new grads may be considered.
