Fairview Health Services is seeking a Nurse Practitioner (NP)to join a primary care practice at Fairview Clinics - Elk River.

The NP, in collaboration with physicians and part of an interdisciplinary team, engages in independent decision making abouthealth care needs of their own patient panel. Assess patients, order and interpretdiagnostic tests, make diagnoses, and initiate and manage treatment plans - including prescribing medications. While maintaining afocus on health promotion and disease prevention, the NP will manage chronic disease conditions ensuring continuity of care for individuals in the communities served.

Patient encounters average 20-22 patients per day.
Onsite services include:pharmacy, lab, x-ray, mammography, and ultrasound.
Onsite specialties include: Ob/Gyn, General Surgery, Pediatrics, Sports Medicine, Rheumatology, Orthopedics, Podiatry, Allergy, Urology
We have a fully integrated Electronic Medical Record (EPIC).
Practitioners work an with expectation of 36 patient contact hours per week.

As part of our participation in the Pioneer Accountable Care Organization (ACO), the physicians use a team approach in managing clinical care. The site is using a model of care, Care Model Innovation, for their patients which actively looks at and changes clinical processes to maximize efficiency, resources, and clinical care.

Position Qualifications:

A qualified candidate will be a graduate of an accredited school of nursing and formal Nurse Practitioner training program. Have a current MN APRN license and be approved or eligible by the Minnesota Board of Nursing for prescription writing privileges. Be eligible or certified through ANCC/AANP and meet threshold criteria/qualifications for Credentialing and Privileges. Ability to obtain and/or maintain DEA certification for the State of Minnesota.

A minimum of 2 years experience as an NP is preferred.
