Fairview Health Services seeks palliative medicine physician to join its growing MHealth Palliative Care Program.

The Palliative Care Program at the University Minnesota Medical Center, one of the first palliative care programs in Minnesota, is developing new models of collaboration with our oncology and cardiovascular colleagues, and growing our services across our integrated academic health system. The program has been designated as a Palliative Care Leadership Center by the Center to Advance Palliative Care since 2003 andincludes interdisciplinary inpatient services and outpatient clinics.

Highlights of this opportunity include:

Provide palliative medicine consultation and education for our inpatient and outpatient clinical programs at the University of Minnesota Medical Center in Minneapolis with the opportunity to practice at a large community hospital, Fairview Southdale Hospital in Edina, MN.

Join a full interdisciplinary team of palliative care social workers, chaplains and advanced practice nurses. In addition to contributing to a clinical program that includes six palliative medicine physicians and four APRNs,.

Serve as faculty for an ACGME accredited palliative medicine fellowship, and work with residents and medical students

Increase interdepartmental collaboration to further enhance and develop the palliative care program population health initiatives.

We have a fully integrated Electronic Medical Record (EPIC).

Must be board certified/board eligible in a primary specialty (internal medicines, family medicine, etc) leading to certification in Hospice and Palliative Medicine.

Preferred Qualifications:

Candidates who have completed a palliative medicine fellowship are preferred.
