University of Minnesota Physicians seeks a Nurse Practitioner (0.8-1.0 FTE) to join the team at the NP Clinic as well as providing coverage at the additional outreach locations. The nurse practitioner engages in independent decision making about health care needs and provides health care services to individuals. The Nurse Practitioner obtains patient histories, performs physical and psychological health assessments, provides patient and family education and counseling, ensures continuity of health care services, and manages acute and chronic disease conditions.

The University of Minnesota School of Nursing opened its first nurse-led clinic inApril 2015 in downtown Minneapolis. This primary care clinicis located across the street from the new stadium and light rail station and serves the neighboring community including the residents of the adjacent housing facility run by RS Eden. Managed and staffed by certified nurse practitioners, the clinic serves people of all ages with wellness care, treatment of routine and chronic conditions and provide referrals as needed andalso serves as a clinical rotation site for nursing, pharmacy and other health profession students from the University of Minnesota. The clinic will be expanding it's scope of services by providing care at additional practice locations including two mental health facilities, a treatment center and a senior living facility.

Job Qualifications:

Graduate of an accredited school of nursing and formal family nurse practitioner training program and experience with chronic disease management.

Current MN APRN license, Approved or eligible by the Minnesota Board of Nursing for prescription writing privileges. Certified through ANCC or AANPor eligible for ANCC or AANPnational certification exam. Meets threshold criteria/qualifications for Credentialing and Privileging.

Preferred Qualifications:

A minimum of3-5 years of experience as an NP in Primary Care. Leadership experience desirable.
