Nurse Practitioner Opening Care Navigator Role

We seek a dynamic Nurse Practitioner with experience in Internal Medicine or Geriatrics who has a passion working with a small population of complex senior patients
The candidate needs to have high empathy and a commitment to deliver excellent quality and service as part of the Stanford Health Care Team
The incumbent must have proven skills in effective outreach; must have pleasant, direct, and clear communication
The Care Navigator is the primary point of contact for a patient in the Care Navigator Program throughout the patients care experience which includes the onboarding, episodic service, and standard follow-up of the patient care experience
The Care Navigator is expected to learn, use, and apply principles relating to Lean/Six Sigma, population management, quality principles and practices, motivational interviewing and care pathways in their daily work
Must be able to use standard health care process improvement methodologies (CQI, Lean, PDSA).
Covering the geographic region of the 80/880 Corridor with your primary hospital at Eden Medical Center. Anticipated Patient Panel of ~75 lives.
This position will require you to provide care in a number of different settings including home visits, skilled nursing facilities, hospital visits, and step-down acute care facilities.
