Palliative Medicine Physician - UnityPoint Clinic "Fort Dodge"

UnityPoint Clinic is actively recruiting for a Palliative Medicine Physician to join our team in Fort Dodge, IA.

Overview:
Outpatient/ Inpatient
Established and Nationally Recognized Practice
Current Providers (1 Physician/ 3 ARNPs)
Experienced staff, both clinical and non-clinical who know the patients extremely well
Quality of Life

Practice Details:
Providers rotate community days and see 6-8 patients per day
Clinic patients = 4-7 per day
Inpatient consults during the day = 5-12 (including ER and cancer center patients)
Call is split 1:2 and is only by phone call
Onsite LTC facility clinic
Provides advanced care to acutely ill patients living with advanced chronic illness, or potentially life-limiting conditions by developing a plan of care based on assessment, examination and selected diagnostic procedures within his/her scope of practice.
Collaborates with the attending physician or palliative care medical director as necessary to formulate a diagnosis and/or appropriate referral.
Works with the Medical Director and Manager to assure that clinical, quality, financial, and outcome results are efficient and compliant with national standards.
