UnityPoint Clinic is actively recruiting for a Nurse Practitioner to join our Skilled Nursing Facility team in Madison, Wisconsin.

Overview:

New outpatient program in Madison, WI
One nurse practitioner position supported by a program medical director in collaboration with primary care physicians
Two years current clinical experience in geriatric or post-acute care
Experience in chronic disease management and palliative are preferred

Practice details:

Average 10 patients/day
Monday-Friday hours between 8:00am and 5:30pm
No weekend or holiday call requirements at this time
Travel required to partner SNF's in the Dane county area
