OSF St. Mary Medical Center is seeking an additional nurse practitioner within the Galesburg market.
Responsibilities include nursing home rounds and assisting with patient and staff education. Perform rounds on Skilled Nursing Facility practice patients.
Participation in call via phone and faxed communication in rotation with APPs within the practice. Home Care Triage staff triage communication before sending onto APP. Physician back-up is always available to APP on call.
Experienced and board certified nurse practitioner or physician assistant is desired.

Epic electronic health record is utilized in OSF inpatient and outpatient settings supporting continuity of care system wide.
