HealthPartners is actively recruiting full-time, part-time or on-call board-certified physicians with an interest and experience in geriatric medicine to join our well-established team in post hospital transitional care. A variety of positions are available in the Twin Cities metropolitan area of Minnesota. This physician would have collaborative practice at all sites with a geriatric nurse practitioner, and manage a census of patients requiring both medical management and monitoring as they progress in rehabilitation after their acute hospital stay. There is also significant opportunities for work in nursing home, assisted living and home based care as well as collaboration with our programs for palliative and end of life care. Nursing Home medical director opportunities also could be part of this role. This position is the ideal opportunity to practice full scope of geriatric medicine in a community arena. The position will also allow the physician to partner with an existing group of dedicated geriatricians and nurse practitioners providing this similar model of care throughout the Minneapolis/ St Paul metro area.
