OSF Medical Group is seeking an Advanced Practice Provider Supervisor (nurse practitioner or physician assistant) for our Skilled Nursing Facility (SNF).
Candidates must have strong oral and written communication skills and the ability to successfully lead and motivate a workforce.
Candidates should have 5 years experience in the clinical delivery of medical care. Experience in leadership in an integrated delivery health system is preferred. Active participation in medical group management education is desirable.

Responsibilities include:
o Communicating, implementing and monitoring the operational objectives of the SNF practice and ensures the provision of quality care delivery and standardized, efficient operations in outpatient palliative care.
o Responsible for smooth transitions of care into and out of the SNF Practice. Works collaboratively with the SNF Director of Operations to ensure all patient referrals and admissions conform with standards and policies.
o Responsible for providing an extraordinary patient experience in SNF. Actively reviews patient satisfaction results for SNF and is knowledgeable of methods and tactics to achieve extraordinary results.
o Participates in the development and execution of the overall provider recruitment and on boarding initiatives for the regions to ensure needs are met.
o Works in collaboration with the Director, SNF to understand and oversee accreditation processes and monitors compliance across regions.
o Actively participates in leadership meetings and serves on various committees and councils as requested.

The OSF Skilled Nursing Facility Practice is APP driven and physician led. Presently, it is comprised of 21 skilled nursing facilities/transitional care units and swing beds throughout the OSF ministry.
