The Department of Veterans Affairs Portland Health Care System (VAPORHCS) is recruiting for Primary Care providers. VAPORHCS is the fastest growing level 1A tertiary VA Hospital in the Nation. VAPORHCS consists of the main tertiary care medical center overlooking the city of Portland, the Vancouver Campus in Vancouver, WA, and ten outpatient clinics across Central and Northwest Oregon. VAPORHCS provides a full continuum of inpatient, outpatient, long-term, and emergent care, serving nearly 90,000 veterans annually. VAPORHCS is proud to host 12 national-level Centers of Excellence that lead research efforts across a wide spectrum of medical specialties. Primary Care providers are located at one of thirteen Community Based Outpatient Clinics (CBOCs) or Outreach clinics. Primary Care providers are accountable to the Group Practice Manager at their clinic location, who works with the Division Clinical Director. The Primary Care Division provides care to more than 63,000 Veterans, with most requiring chronic disease management. The ideal candidates will have previous experience with panel management and managing chronic conditions. To accomplish its mission, the Service includes more than 380 FTE, with a mix of more than 60 physicians and 10 nurse practitioners. Primary Care operates under the Patient Aligned Care Teams (PACT) model of care, which consists of one provider and three clinical and admin support staff. This allows for relationship building and continuity of care amongst Veterans and their clinical team. We seek a board certified or board eligible Internal Medicine or Family Practice physician, or certified Nurse Practitioner. VAPORHCS has a close relationship with our academic affiliate, Oregon Health & Science University (OHSU), to which it is physically connected by a sky bridge. The VAPORHCS Primary Care Division has a unique relationship with the Department of Hospital and Specialty Medicine and OHSU, with an Internal Medicine Resident clinic. VAPORHCS has a robust research program, with over $32 million in grant funding. VAPORHCS is a leader in high quality, cost effective clinical care, having been recently recognized as among the best by CMS Hospital Compare, recipient of a 2013 Joint Commission Top Performer Award on Key Quality Measures and a 2013 National Center for Patient Safety Cornerstone Silver Level Award. VAPORHCS is also a Magnet Recognition Program Facility.
