The Division of Neonatology at Duke University Medical Center has a strong reputation for clinical excellence, and we pride ourselves on training exceptional clinical scientists. Our fellowship program recruits individuals interested in outstanding clinical care and multidisciplinary research. We offer a wide variety of research and training opportunities that are tailored to each incoming fellow.

Clinical Training: A new neonatal intensive care unit at Duke was recently opened, and provides state-of-the art neonatal therapies. Our new 50-bed unit is directly adjacent to a recently renovated labor and delivery ward that services over 7000 deliveries per year. This environment fosters excellent clinical training in a wide variety of neonatal conditions. Duke offers both primary and sub-specialty care to North Carolina and much of the Southeast United States. We also have an active Life Flight Transport Team that services the surrounding communities and states.

Research Training: The Duke neonatology fellowship program is based on the premise that neonatologists carry out important functions in neonatal research: 1. Describing and categorizing disease, 2. Elucidating basic disease mechanisms, 3. Developing potential therapies, testing these therapies in clinical trials, 4. Educating practitioners in the implementation of new therapies.

We believe that effective training is supported in a milieu in which scientists with a broad variety of interests and backgrounds come together to share ideas and provide one-on-one mentoring of junior physician-scientists. Such a milieu exists in the neonatal training program at Duke and the Neonatal-Perinatal Research Institute (NPRI), a multidisciplinary group of basic, clinical and health care researchers dedicated to the study of developmental biology and health care problems in the neonate. The training of physician-scientists to carry out outstanding research is an integral part of the mission of the Medical Center, the Division of Neonatal-Perinatal Medicine, and the NPRI.

Recently, Duke became an active member of the NICHD, Neonatal Research Network. This network is made up of 16 academic Divisions of Neonatology and ICNs dedicated to improving the quality of care for sick infants. Neonatal fellows are encouraged to take part in clinical research under the auspice of the Neonatal Network.

Many of our fellows have chosen alternative academic pursuits during their training. Masters level programs are available in Clinical Research and Genomics as well as disciplines as varied as Biomedical Engineering, Medical Informatics, as well as Medical Economics and Public Policy.

It is also our belief that outstanding physicians, depending on their aspirations and interests, can make significant contributions to health care policy, medical ethics or medical informatics in the context of a medical research career. Accordingly, we have developed a three-track approach: Physician Scientist Track (Basic Research); Clinical Scientist Track (Clinical Research); Clinical Scholar Track (Educator Clinical). These support traditional basic and clinical research education, as well as training in health care policy and medical informatics in the context of a medical research career.

An unexpected position is available for our Pediatric Endocrinology & Diabetes Fellowship to begin July 2006.

The major goal of this subspecialty training program in pediatric endocrinology, diabetes and metabolism is to provide trainees with the medical knowledge, clinical exposure, research experience, and intellectual tools to diagnose and manage endocrine disorders affecting infants, children, adolescents, and young adults. Our program provides a progressive learning experience in clinical and research endocrinology/diabetes/metabolism. Trainees are given increasing responsibility as they progress through the program and demonstrate their expanding knowledge base and expertise in the diagnosis and management of endocrine disease. Under the direct supervision of the attending physician, trainees serve as leaders of the endocrine 'team', which consists of the trainee, pediatric residents and medical students. Our program emphasizes a scholarly approach to diagnosis, patient management, and research. Self-instruction and continual improvement (Practice-based learning) are expected of the trainee.

The faculty members of the Division of Pediatric Endocrinology, Diabetes, and Metabolism have diverse interests, are actively involved in and well funded for clinical and basic research and training, and are widely interactive with other components of the Children's Hospital of Pittsburgh and the University of Pittsburgh. A post-doctoral training program in pediatric endocrinology, diabetes, and metabolism at the Children's Hospital of Pittsburgh has existed continuously since 1947. This training program has an extraordinary track record in post-doctoral research training, and in launching the academic careers of its trainees. Thus, this teaching environment will equip trainees to become strong clinicians, outstanding educators, and skilled investigators. The program is divided into two major rotations. The first rotation, lasting one year, focuses primarily on clinical pediatric endocrinology and diabetes. The second rotation, lasting two years, consists of a mentored research experience. Staff: Our Section of Endocrinology clinical and basic science faculty includes ten M.D.'s and five Ph.D.'s. Dr. Wayne Moore is our Chief of Endocrinology. Dr. Miles Yu, Dr. Jill Jacobson, Dr. Jadranka Popovic, Dr. Figen Ugrasbul, Dr. Carol Huseman, Dr. Kurt Midyett, Dr. Joseph Cernich, Dr. Ellen Kim, and Dr. Angela Turpin complete the clinical training section. History: The program has been in existence since 1985 and is certified by ACGME program. The fellowship has focused on clinical training in the past, as we have one of the largest diabetes and growth hormone treatment programs in the United States. The research component has developed recently with the hospital's commitment to excellence in research. Our training program offers the opportunity for fellows to become proficient in clinical, basic and translational research.

Research: Our section has two active laboratories, both with a history of NIH funding, Dr. Jacobson's lab and Dr. Moore's lab. Dr. Jacobson's lab is actively involved in studying the gender differences in immunity and autoimmunity. It focuses on the role of G proteins in gender differences and the effects of hormones on gender differences in expression of these G proteins. Dr. Moore's lab is involved with diabetes research, including studies of pancreatic islet transplantation and prevention of autoimmune diabetes with anti-CD154 antibodies. These labs are located in the UMKC School of Medicine building, contiguous with our endocrine section offices. Our section has 21 active research grants totaling more than 1.3 million dollars.

Clinical: Clinically, we staff more than 7,000 outpatient visits and more than 400 inpatient admissions per years with the entire spectrum of endocrine diseases. We staff six outreach clinics in Kansas and Missouri. We are a referral center for a wide range in the Midwest. We see patients from Kansas, Missouri, Iowa, South Dakota, Arkansas, Nebraska and Oklahoma.

Teaching: We have 13 regularly scheduled conferences and lecture series. Additional lectures in statistics have been set up expressly for fellows.

A flexible three-year fellowship program in Pediatric Hematology/Oncology is available for qualified physicians who have completed residency training in Pediatrics and are Board-certified or Board-eligible. The goal of the training program is to provide broad based clinical as well as laboratory skills necessary to attain a future pediatric hematology/oncology appointment in an academic institution.

The laboratory core curriculum includes rotations through specialized laboratories devoted to Hematologic Pathology, Coagulation, Blood Banking, and Red Cell Physiology. Elective rotations are available in other laboratories in the areas of Pharmacology, Biochemistry, Immunology, Microbiology, Nuclear Medicine, and Experimental Oncology.

The clinical program provides exposure to a large number of American and foreign patients with a broad range of hematologic and oncologic problems. In association with the Mayo Comprehensive Hemophilia Diagnostic and Treatment Center and the Mayo Comprehensive Cancer Center, the fellow will participate in clinical research and laboratory research activities.

Mayo also has a separate Pediatric BMT Program and is approved for unrelated donor transplants. The fellow would get experience in taking care of different types of transplants including autologous, allogeneic, related and unrelated and haploidentical transplants.

Clinical and laboratory research opportunities are available in almost any facet of Hematology or Oncology. Mayo participates actively in the activities of the Children's Oncology Group. Fellows are exposed to clinical protocol development, data collection and analysis, and are provided with opportunities to become familiar with cooperative group research activities. The superb electronic record system at Mayo facilitates multidisciplinary patient care and research. Fellows are expected to present results of research at regional and national meetings.

Attendance and presentations at conferences facilitate an excellent knowledge base at the completion of the program. Conferences include: weekly Hematology conference, Hematology and Oncology Journal Club, biweekly Hematology and Oncology case conference, weekly research presentation meetings of the Pediatric Hematology/Oncology Division, and weekly Pediatric Grand Rounds. The fellow can also enroll in other courses for credit within the Mayo Graduate School of Medicine, which cover such topics as Medical Statistics, Cancer Biology, Biochemistry, Immunology, and Pharmacology. Fellows also attend the monthly dinner meetings of the Oncology Society, Genetics Society, and the Hematology Society. These feature outstanding lectures by members of Mayo staff and visiting faculty.

During the first year of the program the fellow is assigned to the inpatient and outpatient clinical services and to laboratories with clinical orientation. During the first year the fellow is encouraged to plan and develop a research project which may require 18 months or more for completion. The second and third year are predominantly research years, later in the fellowship the fellows will assume increased responsibility in consultations and return visits. For fellows seeking to enter a career in laboratory-based research, a number of opportunities are available to compete for intramural and extra mural funding to support an additional year of laboratory-based research. A masters program is also available for fellows interested in a clinical research program.

Thank you for your interest in our Pediatric Nephrology Training Program. Established in 1966, it is designed to provide the fellow with a wide experience in all areas of Pediatric Nephrology. Our activities are as follows:

*Clinical. Children's Hospital at Washington University Medical Center in St. Louis draws patients from a wide region in the Midwest, including many with a variety of renal diseases. Our program provides complete facilities for diagnostic evaluation, inpatient and outpatient management and follow-up. There is an average of 1,500 renal outpatient visits made per year. The usual annual inpatient census is 100-200 with daily census ranging up to 15 patients at any one time. The fellows supervise the management of renal patients under the direction of a faculty member. Routine care and procedures are performed by the House Staff of St. Louis Children's Hospital.

End stage renal disease care is provided through several modalities. We have a chronic hemodialysis unit for children. Patients are also being dialyzed at home and in out-state satellite units. An active continuous ambulatory peritoneal dialysis (CAPD and CCPD) program is also maintained. The techniques of peritoneal dialysis, hemodialysis, continuous arteriovenous hemofiltration and percutaneous renal biopsy are learned and performed by the fellows under supervision of the faculty. The Renal Transplant Service is run jointly by Transplant Surgery and Pediatric Nephrology. Each fellow has an opportunity to manage several patients who have received renal transplant.

*Teaching. The renal fellows are an integral part of the teaching program for both the junior and senior medical students at Washington University and the House Staff at St. Louis Children's Hospital. Clinical aspects of pediatric Nephrology, as well as pathophysiology and physiology, are regularly discussed during rounds on the patients.

In addition, the fellow has ample opportunity to continue his own post-graduate education program. We hold joint conferences with the Adult Renal Division: a ) Renal Grand Rounds in which faculty members present topics in detail; b) weekly research conferences in which current work is presented to the group and analyzed; and c) renal pathology conferences in which biopsy material is presented and discussed. Radiology conferences are held weekly in conjunction with the Departments of Radiology and Urology. Finally, our group frequently participates in conferences and rounds in other areas of pediatrics.

The education program is greatly enhanced by its close association with the Adult Renal Division, headed by Dr. Marc R. Hammerman, with the Pediatric Urology Program, headed by Dr. Doug Coplen, and with Dr. Helen Liapis in Renal Pathology.

* Research. We are currently active in several areas of clinical investigation, including mechanisms and treatment of renal osteodystrophy, treatment of pediatric hypertension and multi-factorial long-term outcomes of renal transplantation. We would like to emphasize that as a result of the association between our program, the Adult Nephrology programs, and other services at Washington University, the pediatric fellows have an opportunity to work with well-known specialists who have a wide variety of interests.

Duration: The duration of the training program by mandate of the American Board of Pediatrics is 3 years minimum. The first year is dedicated to clinical training and the second and third year are primarily dedicated to research training plus continuity clinic for longitudinal care of chronically ill rheumatology patients. An additional fourth and fifth year may be chosen by individual fellows when additional research expertise is selected.

Prerequisite Training/Selection Criteria: The prerequisite training includes a 3-year residency in an ACGME approved pediatric residency program or a 4-year medicine pediatrics residency program (none of the med/ped residencies are ACGME approved). The special reduced duration programs are taken under special consideration, such as short tracking for two years of pediatric residency in special situations; if the resident has accomplished all the requirements as required by the American Board of Pediatrics.

Goals and Objectives for Training: The pediatric rheumatology subspecialty residency training program is designed and implemented with two goals that will prepare pediatricians for an academic pediatric rheumatology career. We have designed this program, first, to prepare our subspecialty residents in the diagnosis and treatment of children, adolescents, and young adults with complex, acute and chronic rheumatologic diseases. The second goal is to prepare the subspecialty residents for a career in biomedical science as it relates to pediatric rheumatology. These goals were developed in the context of a fellowship retreat at which all of the faculty in the division agreed on the goals and the schedule of the training program. The faculty added since that retreat were fellows during and after the time of the retreat and are well-versed in the divisional goals of the fellowship program. We have developed the essential components to satisfy both the clinical and research goals of the residency.
